Newman Dermatology is required by law to:

  • Maintain the privacy of your Protected Health Information (PHI)

  • Provide this Notice describing our legal duties and privacy practices

  • Notify you if the privacy or security of your PHI is ever compromised

  • Follow the terms of this Notice while it is in effect

Request that we communicate with you in a specific way (e.g., via a specific phone number).

Get a List of Certain Disclosures

Receive an accounting of disclosures of your PHI made outside of treatment, payment, or operations.

Right to File a Complaint

If you believe your privacy rights have been violated, you can file a complaint with:

  • Our office (contact information below), or

  • The U.S. Department of Health and Human Services (HHS)

You will not be penalized for filing a complaint.
